EHR Scope of Data

Chart EHR data generally include patient demographics, laboratory (clinical, cytology, pathology), diagnosis, problem list, medication, immunization history, procedures, diagnostic/imaging data, vital signs, allergy history, social history, family history and health-care provider details including physician and non-physician data. Based on the data extracted from the EHR, standard of care (SOC) queries have been developed to retrieve information that indicates the treatment rendered to patients or results of diagnostic services provided to patients. If the query relates to the value of a laboratory result, the query evaluates and reports on the specific value(s) identified in the query. Measures may be a count of the item, the clinical value of the item, or an average.


CQI Description

Electronic Medical Record (EHR) data is routinely collected and updated during the course of a patient’s office visit, including relevant data supplied by patients. The PSIMedica CareIndex® Quality Indicator (CQI) module is configured based on the full integration of health care services data maintained in an electronic medical record (EHR)including eligibility information, Inpatient and outpatient encounters, patient demographic data, primary and other physician information, laboratory results, family & social history, problem list, and pharmaceuticals and much more.

CQI Disease Profile Capability

Chart CQI is oriented to the analysis of diseases, and provider specialty populations. Patients are assigned to these populations based on laboratory test results and/or ICD-9 diagnosis codes and/or problem list descriptions of conditions derived from the EHR data. Key variables e.g., LDL test, colonoscopy procedure, or specific medications are defined and employed in the CQI analytics. A set of clinical and/or administrative healthcare questions and measures are associated with each identified population. The User selects the questions for which a response will display regarding the population. The responses to the questions display simultaneously using a multi-query retrieval process.

The CQI module operates at an aggregate disease/disorder level. From the aggregate level, you can drill-down to the patient level. The User may “drill through” any selected cell into the underlying individual patient data. The User may identify or de-identify Individual patient data depending upon the security privileges of the User.
